Project my database to collect random fields values

My page would have two ranks admin and user.

First admin log in to page and fill the form 1) Write own field names. For ex. Name, Age, Hair Color

After press submit page return link with orginal key for ex. http://ift.tt/1KUoJcR

And next admin give this link to people and they when opening this link must fill the form with values which are submited by admin.

Of course they can write what they want in values. Own hair colour age etc.

Admin can view this data by key in admin panel.

The same admin or second one might enter into admin page and generate own field and get own key to send it to people.

Question: How to project mysql DB I mean tables and columns to store this data without redundant. I dont think that one table per one orginal key is a good way to solve problem.
