Insert a Database into android project

The database will be already created and NOT from the app in runtime.

I want to answer me in:

1. Where I must put the file of the database in my android project(The path).
2. What file format must be the database like (.db, .sqlLite) etc.
3. I want the database PRIVATE and only access from my app.
4. How can access the database from my Java code(code example).
