bTreeInitPage() returns Error code 11 - What caused it?

I have looked through all the pages related to corrupted sqlite databases (How To and Recovery Options), I know how to recover any data that is recoverable, but my goal is to prevent this in the first place.

I have verified that there were no power or program failures/crashes before the error began to occur (Error in question is SQL Error: SQL logic error or missing database when querying certain data...NOT ALL DATA IS AFFECTED!!)

The integrity check tells me:

Page 889: btreeInitPage() returns error code 11
Error: database disk image is malformed

I'm neither certain what that means, how that happened, nor what I can do to prevent it in the future. Please assist?
