XAMPP: Phone Book sample app not working

I am very new to PHP and SQLite. I was just testing some of XAMPP's pre-loaded sample apps. One of which is Phone Book under Php.

Here is the unmodified pastebin of phonebook.php. I put it in pastebin, so people can see the line numbers.

How do I get the sample app to work?
